Testosterone injection cost: what cypionate costs online each month

What testosterone injections cost online: monthly price by program, compounded versus generic cypionate, and the labs, visit and membership fees that change it.

By Julian Caraulani · Updated October 31, 2026

The cheapest testosterone injection plan we track is $79 a month at PeterMD, and that is a prepaid, compounded plan. Generic testosterone cypionate costs more at programs that sell both. The monthly figure also depends on labs, the visit and any membership, so compare the all-in price on our TRT price table.

How much do monthly testosterone injections cost?

Online programs price injections as a monthly figure, but what that figure covers differs. Our headline number always adds any required membership to the medication. The table shows the lowest injection plan each program publishes, what is in it and where we read it.

Testosterone cypionate price: generic versus compounded

Testosterone cypionate is the most common injectable. The FDA-approved label describes it as a product that "is available in one strength, 200 mg/mL testosterone cypionate" (DailyMed label). That is the generic drug. A compounded injection is made by a pharmacy for a specific patient and, in the FDA's words, compounded drugs are not FDA-approved (FDA compounding Q&A).

The two are not always priced alike. Hims lists a compounded injection and a generic cypionate option separately:

Hims option Medication + membership Monthly all-in
Compounded injectable $79 + $49 $128
Generic testosterone cypionate $159 + $49 $208

Hims says on its testosterone page that the membership fee is paid at checkout and the medication cost is not included (hims.com). So the cheapest advertised injection is often compounded, and a like-for-like comparison against generic cypionate should use the generic price.

What changes the price of a testosterone injection

  • Labs. The Endocrine Society guideline says "Clinicians should monitor men receiving T therapy using a standardized plan" (Bhasin et al., J Clin Endocrinol Metab 2018). Programs that bill each test add that cost every year, and those that include labs look higher up front.
  • Prepaying. The lowest rates need a 6- or 12-month commitment. Month to month rates at PeterMD and Maximus are well above the prepaid ones.
  • Membership and consult fees. A medication-only price can leave out a required monthly membership or a first-visit fee.
  • Supplies and shipping. Some programs include needles and syringes and some do not state it.
  • Your state. Availability and some prices are set by state.

Who can prescribe testosterone injections

Testosterone is a controlled drug. The label states that "Testosterone Cypionate Injection contains testosterone, a Schedule III controlled substance in the Controlled Substances Act" (DailyMed label). It is prescribed for men with low testosterone confirmed by testing. The label says safety and efficacy in "age-related hypogonadism" have not been established, so a program should test before treating. A price for injections is the price after you qualify, not before. Read how it works in our guide to TRT online.

Is a local doctor cheaper than an online program?

Sometimes. A 2022 study compared a year of injections through three telemedicine platforms with care at one hospital center and found "the cost of 12 months of intramuscular testosterone treatment was highest via the telemedicine platforms" (Jesse et al., J Sex Med 2022). The evidence is thin, with three platforms and one center. If your insurance pays for testosterone, get a quote from your own doctor first, and read whether TRT is covered by insurance.

FAQ

How much do testosterone injections cost?

Online, the lowest injection plan we track is $79 a month at PeterMD, on a prepaid plan. Programs that bundle labs and the visit run higher. The all-in figure, not the medication price, is the one to compare, and the price table holds current numbers.

How much do monthly testosterone injections cost?

It depends on what the plan includes. The table above shows programs from the low end to about $219 a month where a membership is required. Check whether labs, shipping, supplies and a first consult are inside the figure before you compare two programs.

What is the testosterone cypionate price?

Cypionate is the generic injectable, and online prices range with the program. Hims lists generic cypionate at a higher monthly total than its compounded option, as shown above. Compounded versions are usually cheaper but are not FDA-approved, so compare generic against generic for a fair price.

Why are some testosterone injections so much cheaper than others?

Low prices usually come from prepaying six or twelve months, using compounded medication, or leaving labs out of the price. A cheap monthly figure can end up costing more over a year once paid labs are added. Compare the first-year total, as our cheapest-TRT guide does.

Does insurance cover testosterone injections?

Sometimes, when low testosterone is diagnosed by testing and the plan covers it, but many online programs are cash-pay. Coverage varies by plan, and prior authorization is common. Ask your insurer, and see our guide on whether TRT is covered by insurance for what we found.

Can I pay for testosterone injections with an HSA or FSA?

Often yes for the prescription. IRS Publication 502 says "You can include in medical expenses amounts you pay for prescribed medicines and drugs" (IRS Publication 502). Whether a membership fee counts depends on your plan, so keep itemized receipts.
