Does Blue Cross Blue Shield cover TRT? What three plans publish

Blue Cross Blue Shield has no single TRT policy. See what the Federal Employee Program, Michigan and Tennessee Blue plans publish, and the cash fallback.

By Julian Caraulani · Updated November 2, 2026

It depends on which Blue plan you hold. Blue Cross Blue Shield is not one insurer, and the plans we could read all want a hypogonadism diagnosis with low morning tests, for example "Two morning total testosterone levels less than 300 ng/dL on different days" (BCBSA Federal Employee Program). Online TRT programs are mostly cash pay from $79 a month all in.

This guide reads three published Blue policies: the Federal Employee Program (FEP) topical testosterone policy, a Blue Cross Blue Shield of Michigan policy for Aveed and Testopel, and a Blue Cross Blue Shield of Tennessee policy for Testopel pellets. They cover different products, so none is a rule for every Blue plan. For the wider picture, see whether TRT is covered by insurance.

Why there is no single BCBS answer

Each Blue company writes its own medical policies, and its own plan contract governs. Tennessee's policy says so directly: "If there is a conflict between the Medical Policy and a health plan or government program (e.g., TennCare), the express terms of the health plan or government program will govern" (BlueCross BlueShield of Tennessee, read November 2, 2026). Michigan adds that its drug policies "are not to be used to determine benefits or reimbursement" (BCBSM, read November 2, 2026).

So the policies below show what Blue plans tend to ask for. Your member services line gives the answer for your plan.

What the Federal Employee Program asks for

The FEP policy, effective July 1, 2026, covers gels and patches such as AndroGel and Androderm. Prior approval needs a "Deficiency of testosterone (hypogonadism)" and all of these: two morning levels under 300 ng/dL on different days, a baseline PSA under 4 ng/ml for patients over 40, no current prostate cancer, hematocrit under 54%, a cardiovascular risk evaluation, no untreated sleep apnea, and no second testosterone product (BCBSA FEP policy 5.30.031, read November 2, 2026).

The policy lists injectables and implants in a separate policy, which we did not read. Renewal asks for a total testosterone level of 800 ng/dL or less and checks every 12 months.

What Blue Cross Blue Shield of Michigan asks for

Michigan's policy covers the long-acting injection Aveed and Testopel pellets, not routine cypionate shots. Coverage needs "Documentation of at least TWO signs/symptoms of testosterone deficiency" and a diagnosis of male hypogonadism. Aveed or Testopel also require trial and failure, contraindication or intolerance of generic Depo-Testosterone or generic Delatestryl (BCBSM, P&T date 12/04/2025).

The background section matches clinical guidance: diagnosis "should be confirmed by serum testosterone measurement in the morning on 2 or more separate days which shows concentrations that are below the normal range."

What Blue Cross Blue Shield of Tennessee asks for

For Testopel, Tennessee authorizes use for primary or hypogonadotropic hypogonadism when "the patient has at least TWO confirmed low morning testosterone levels according to current practice guidelines or your standard lab reference values" (BlueCross BlueShield of Tennessee). The request must not be for age-related hypogonadism, and the policy notes that safety and efficacy in "age-related hypogonadism" have not been established.

That echoes the Endocrine Society, which recommends "confirming the diagnosis by repeating the measurement of morning fasting total T concentrations" (Bhasin et al., J Clin Endocrinol Metab 2018).

Does a Blue plan cover online TRT programs?

Do not assume so. The programs we track mostly sell on a published cash price and do not bill insurers. Male Excel states its approach "is not covered by insurance" (maleexcel.com). Many dispense compounded testosterone, which the FDA says is not FDA-approved (FDA), so the policies above, written for approved brands, would not clearly apply. Ask the program first.

Blue plan or cash: how to decide

A 2022 study found "the cost of 12 months of intramuscular testosterone treatment was highest via the telemedicine platforms" among the models compared (Jesse et al., J Sex Med 2022). It covered three platforms and one hospital, so it shows a direction only. If your plan approves you, local care may cost less; if you are denied or have a high deductible, cash can be simpler. FAQ

Does Blue Cross Blue Shield cover TRT?

Often, with conditions. The Blue plans we read cover testosterone products for diagnosed hypogonadism, usually after prior approval and two low morning testosterone results. Each Blue company sets its own rules, and your plan contract governs, so call the number on your member card before filling a prescription.

Does BCBS require prior authorization for testosterone?

In all three policies we read, yes. FEP lists "Prior-Approval Requirements" for topical testosterone, Michigan requires chart notes and documentation for Aveed and Testopel, and Tennessee grants authorization only when its criteria are met. Plans differ, so ask yours.

What testosterone level does BCBS require?

The Federal Employee Program asks for two morning total testosterone levels below 300 ng/dL on different days. Tennessee refers to current practice guidelines or the lab's standard reference values instead. Michigan does not set a number in its policy text. Your plan may use a different threshold.

Does BCBS cover testosterone for low testosterone from aging?

Tennessee's policy requires that the drug is not prescribed for age-related or late-onset hypogonadism. The FDA label says safety and efficacy in that group are unestablished. A doctor may diagnose another cause, which could meet criteria. Check your plan's wording.

Does BCBS cover testosterone gel or pellets?

Policies are written product by product. FEP covers topical gels and patches with prior approval, Michigan and Tennessee address Aveed and Testopel. Coverage for a specific brand depends on your plan's drug list, so check the formulary.

Can I use Blue Cross Blue Shield for an online TRT program?

Usually not. Most online programs are cash pay and do not bill insurers. If you have an HSA or FSA, those funds may cover eligible costs. Compare programs on our TRT price table.
