Does Cigna cover TRT? What its testosterone policies require

Cigna requires prior authorization for testosterone, with two low early-morning tests. See what its injectable and topical policies ask for and the cash fallback.

Sometimes, with conditions. Cigna's testosterone policies say "Prior Authorization is required for benefit coverage of injectable testosterone" (Cigna IP0351), and the same for oral, topical and nasal products. Online TRT programs are mostly cash pay from $79 a month all in.

This guide reads Cigna's two published drug coverage policies, IP0351 (injectables) and IP0350 (oral, topical, nasal), then compares them with the cash route. For the wider picture, see whether TRT is covered by insurance.

What Cigna requires for a first approval

Both policies use the same test for a new start. The patient needs persistent signs and symptoms of androgen deficiency, plus "each taken in the early morning, on two separate days" testosterone measurements that are both low by the laboratory's reference range (Cigna IP0350, effective June 15, 2026, read November 3, 2026). The policy lists symptoms such as depressed mood, decreased energy, loss of muscle mass, osteoporosis and loss of libido.

Approval is for 1 year. People already on treatment have an easier test: signs and symptoms plus at least one low pretreatment level.

Injectables versus gels and pills

Injectables (IP0351) Oral, topical, nasal (IP0350)
Effective date October 15, 2026 June 15, 2026
Products named Aveed, Azmiro, Testopel, Xyosted Jatenzo, Kyzatrex, Tlando, gels, solution, Natesto
Prior authorization Yes Yes
Two low early-morning levels Yes Yes
Approval length 1 year 1 year

Neither policy names generic testosterone cypionate vials by name as a product it reviews. Azmiro and Xyosted require a prior try of testosterone cypionate or enanthate injection, and several brand gels and capsules require a try of cheaper generic forms first. Which of those steps applies depends on your drug list (Cigna IP0351, read November 3, 2026).

Where the 300 ng/dL number comes from

Cigna's policy cites the American Urological Association: clinicians "should use a total testosterone level below 300 ng/dL as a reasonable cut-off" (Cigna IP0351). Cigna's own approval test, however, uses the lab's reference range rather than a fixed number. The Endocrine Society likewise recommends "confirming the diagnosis by repeating the measurement of morning fasting total T concentrations" (Bhasin et al., J Clin Endocrinol Metab 2018).

Your plan document still decides

Cigna states that "benefit plan document always supersedes the information in the Coverage Policies" (Cigna IP0351). Some employer plans only provide utilization review. Whether a drug is covered under the medical or pharmacy benefit also varies. We found no separate Express Scripts testosterone policy to quote, so ask your plan which criteria it uses.

Does Cigna cover online TRT programs?

Do not assume so. The programs we track mostly sell on a published cash price and do not bill insurers. Male Excel states its approach "is not covered by insurance" (maleexcel.com). Many dispense compounded testosterone, which the FDA says is not FDA-approved (FDA), and Cigna's policies are written for approved brands. Ask the program first.

Cigna or cash: how to decide

A 2022 study found "the cost of 12 months of intramuscular testosterone treatment was highest via the telemedicine platforms" among the models compared (Jesse et al., J Sex Med 2022). It covered three platforms and one hospital, so it shows a direction only. If Cigna approves you, local care may cost less; if you are denied or face a high deductible, cash can be simpler. FAQ

Does Cigna cover TRT?

Often, with conditions. Cigna's policies cover testosterone for diagnosed hypogonadism after prior authorization and two low early-morning tests. Your plan document controls the final answer, and some employer plans differ, so call the number on your member card before filling a prescription.

Does Cigna require prior authorization for testosterone?

Yes. Both published policies state that prior authorization is required, one for injectables and one for oral, topical and nasal products. Approval is for 1 year, and renewal needs continued signs and symptoms plus a documented low level.

What testosterone level does Cigna require?

Cigna asks for two early-morning measurements on separate days that are both low by the laboratory's reference values. It cites the AUA's 300 ng/dL cut-off as guidance, but the approval test itself refers to the lab range. Your plan may apply a different threshold.

Does Cigna cover testosterone cypionate?

The injectable policy we read names Aveed, Azmiro, Testopel and Xyosted, and references generic cypionate as the product other brands must be tried against. Whether plain cypionate is covered, and how, depends on your drug list. Check your formulary or ask your plan.

Does Express Scripts cover testosterone?

We did not find a separate Express Scripts testosterone policy to quote. Cigna's own policies are the published source we read. If your plan uses Express Scripts for pharmacy benefits, ask them for the exact criteria and drug list that apply to you.

Can I use Cigna for an online TRT program?

Usually not. Most online programs are cash pay and do not bill insurers. If you have an HSA or FSA, those funds may cover eligible costs. Compare programs on our TRT price table.
