Does UnitedHealthcare cover testosterone? What its policy requires

UnitedHealthcare covers injectable testosterone for hypogonadism with two low morning tests and a 12-month authorization. See the criteria and the cash fallback.

By Julian Caraulani · Updated November 4, 2026

Sometimes, with conditions. UnitedHealthcare's commercial policy says coverage is "contingent on criteria" for testosterone cypionate, enanthate, pellets and Aveed (UnitedHealthcare policy 2026D0076J). Online TRT programs are mostly cash pay from $79 a month all in.

This guide reads UnitedHealthcare's published testosterone policy, then compares it with the cash route. For the wider picture, see whether TRT is covered by insurance.

What UnitedHealthcare requires for injectable testosterone

The commercial medical benefit drug policy (effective January 1, 2026, read November 4, 2026) covers injectable testosterone and Testopel pellets as medically necessary for hypogonadism. A patient with no history of surgical removal of the testes, panhypopituitarism or a genetic cause must show one of these:

  • Two pre-treatment "early morning serum total testosterone levels less than 300 ng/dL" or below the lab's reference range, taken at separate times.
  • A condition that can alter sex-hormone binding globulin (such as thyroid disorder, HIV, liver disorder, diabetes or obesity) plus one low calculated free or bioavailable testosterone level.
  • For people already on treatment, a follow-up free or bioavailable level drawn within the past 6 or 12 months.

The policy also requires that dosing follow FDA-approved labeling and that "Authorization will be for no more than 12 months" (UnitedHealthcare policy 2026D0076J). The Individual Exchange version, IEXD0076.07, carries the same January 1, 2026 effective date (IEXD0076.07).

What the policy covers, and what it leaves out

Product What the policy says
Testosterone cypionate (Depo-Testosterone) Covered if the criteria are met
Testosterone enanthate Covered if the criteria are met
Testopel pellets Covered if the criteria are met
Aveed (undecanoate) Covered if the criteria are met
Azmiro "typically excluded from coverage"
Compounded testosterone and pellets "not proven or medically necessary for any indication"

The policy is a medical benefit drug policy. It does not list gels, patches or oral capsules, and we did not find a separate OptumRx testosterone policy to quote, so ask your plan about those forms.

Compounded testosterone is not covered

This matters for online programs. The policy states that "Compounded hormone products (e.g., pellets), including but not limited to compounded testosterone, estrogen, and progesterone pellets are not proven or medically necessary for any indication" (UnitedHealthcare policy 2026D0076J). The FDA separately says compounded drugs are not FDA-approved (FDA). Many online programs dispense compounded testosterone, so do not expect UnitedHealthcare to pay for them.

Your plan document still decides

The policy states that "The member specific benefit plan document must be consulted to make coverage decisions for this service" (UnitedHealthcare policy 2026D0076J). State rules can override it. UnitedHealthcare also publishes separate Medicaid community plan versions that differ by state, so a policy you find online may not be yours. The Endocrine Society likewise recommends "confirming the diagnosis by repeating the measurement of morning fasting total T concentrations" (Bhasin et al., J Clin Endocrinol Metab 2018).

UnitedHealthcare or cash: how to decide

A 2022 study found "the cost of 12 months of intramuscular testosterone treatment was highest via the telemedicine platforms" among the models compared (Jesse et al., J Sex Med 2022). It covered three platforms and one hospital, so it shows a direction only. If UnitedHealthcare approves you, local care may cost less; if you are denied or face a high deductible, cash can be simpler. FAQ

Does UnitedHealthcare cover testosterone?

Often, with conditions. The commercial policy covers injectable testosterone and pellets for diagnosed hypogonadism after two low early-morning tests or a qualifying alternative. Your plan document controls the final answer, so call the number on your member card before starting treatment.

Does UnitedHealthcare require prior authorization for TRT?

The policy sets criteria for medical necessity review and limits authorization to no more than 12 months. Whether your plan requires review before each fill depends on your benefit. Ask UnitedHealthcare or your prescriber's office to confirm before the first injection.

What testosterone level does UnitedHealthcare require?

The policy asks for two pre-treatment early-morning total testosterone levels below 300 ng/dL, or below the lab's reference range, taken at separate times. A free or bioavailable level is accepted for people with conditions that alter binding proteins. Your plan may apply different thresholds.

Does Optum Rx cover testosterone?

We did not find an Optum Rx testosterone policy we could quote. The UnitedHealthcare policy we read is a medical benefit drug policy for injectables and pellets. For gels, patches or oral products, check your formulary or ask your plan for the criteria that apply.

Does UnitedHealthcare cover online TRT programs?

Usually not. Most programs are cash pay and do not bill insurers, and UnitedHealthcare's policy excludes compounded testosterone. If you have an HSA or FSA, those funds may cover eligible costs. Compare programs on our TRT price table.

Is there a cash alternative if UnitedHealthcare denies TRT?

Yes. Online programs publish monthly prices, and our table shows each all-in cost. Cash pricing avoids prior authorization but may not count toward your deductible. See how much TRT costs without insurance for the breakdown.
