Does Humana cover TRT? What its published lists show
Humana lists generic testosterone cypionate at Tier 1 on a 2026 Medicare group formulary, with gels needing authorization. See what is covered and the cash fallback.
By Julian Caraulani · Updated November 5, 2026
Sometimes, depending on the plan and the product. On Humana's 2026 Group Plus Medicare formulary, testosterone cypionate intramuscular oil is listed at Tier 1, while testosterone gels carry a prior authorization flag (Humana 2026 Group Medicare list). Online TRT programs are mostly cash pay from $79 a month all in.

This guide reads the Humana documents we could open, says plainly what we could not, and compares coverage with the cash route. For the wider picture, see whether TRT is covered by insurance.
What Humana's published lists show
Humana's pharmacy coverage policy index includes a policy titled "Non-Preferred Testosterone Products," effective 1/1/2025 and last reviewed 4/15/2026 (Humana coverage policy index, read November 5, 2026). The index page is a listing only. The policy text sits behind a disclaimer step our reader could not pass, so we do not quote its criteria here.
What we could read is the 2026 Group Medicare Commonly Prescribed Medications List for the Group Plus formulary (26800), read November 5, 2026. It states that medications in its Tier 1, 2 column "are covered and are the most affordable" and that those in the Tier 3, 4 column "are covered, but at a higher member cost share" (Humana 2026 Group Medicare list).
| Product on the list | How it appears |
|---|---|
| Testosterone cypionate intramuscular oil | Tier 1, no PA flag |
| Testosterone transdermal gel packet and pump | Tier 1, PA flag |
| Androderm patch | Tier 3, PA flag |
| AndroGel 1% packet | Tier 3, PA flag |
| AndroGel 1.62% packet | Tier 4, PA flag |
| Testosterone implant pellet, Testone CIK | In the column that "may require a coverage determination" |
The list says "PA = Prior Authorization." It is a guide, not a complete formulary, and it covers one group plan. Your own formulary can differ.
Injectable Aveed needs authorization on Medicare Advantage
Humana's Medicare Advantage and Dual Eligible Special Needs Plans prior authorization list (effective January 1, 2026) includes Aveed, testosterone undecanoate, billing code J3145 (Humana Medicare prior authorization list, read November 5, 2026). That list is for medical-benefit drugs, so it does not tell you about the pharmacy-benefit cypionate vials above.

What we could not verify
We did not find a Humana commercial testosterone policy with published lab thresholds, so we do not state one. Third-party sites quote a two-test, 300 ng/dL rule for Humana, but they conflict with each other and are not Humana documents. The FDA says compounded drugs are not FDA-approved (FDA), and we found no Humana statement covering compounded testosterone from online programs. Ask Humana before assuming any program is reimbursable.
Clinically, the Endocrine Society recommends "confirming the diagnosis by repeating the measurement of morning fasting total T concentrations" (Bhasin et al., J Clin Endocrinol Metab 2018). Expect any insurer to ask for repeat morning labs.
Humana or cash: how to decide
A 2022 study found "the cost of 12 months of intramuscular testosterone treatment was highest via the telemedicine platforms" among the models compared (Jesse et al., J Sex Med 2022). It covered three platforms and one hospital, so it shows a direction only. If Humana covers generic cypionate on your plan, a local prescriber may cost less; if you are denied or face a high deductible, cash can be simpler. Our TRT cost guide breaks down each fee, and the top-scored program on our best TRT programs page is Maximus. Other insurers: Aetna, Blue Cross Blue Shield, Cigna and UnitedHealthcare.
FAQ
Does Humana cover TRT?
Often for generic injectable testosterone, depending on the plan. A 2026 Humana Medicare group formulary lists testosterone cypionate oil at Tier 1, and gels carry prior authorization flags. Your own formulary controls, so check it or call the number on your member card.
Does Humana require prior authorization for testosterone?
It depends on the product and plan. On the 2026 group list we read, gels and patches are flagged PA, while cypionate oil is not. Aveed appears on the Medicare Advantage authorization list. Confirm with Humana before your first fill.
What testosterone level does Humana require?
We could not read a Humana policy with lab thresholds, so we do not state one. Third-party sites cite a figure, but they are not Humana documents. Ask your prescriber to check your plan's criteria, and expect repeat morning testosterone tests.
Does Humana cover testosterone pellets?
On the 2026 group list we read, the testosterone implant pellet sits in the column that "may require a coverage determination." Coverage is therefore not assured. Ask Humana for the criteria on your plan before scheduling a pellet procedure.
Does Humana cover online TRT programs?
Most online programs are cash pay and do not bill insurers, and we found no Humana statement covering compounded testosterone. If you have an HSA or FSA, those funds may cover eligible costs. Compare programs on our TRT price table.
What if Humana denies testosterone?
Ask your prescriber about an appeal or peer review, and ask Humana for the denial reason in writing. Cash programs avoid authorization. See how much TRT costs without insurance for the breakdown.
